The area is numbed with tumescent fluid, a dilute local anesthetic with epinephrine that also limits bleeding and bruising. Dr. Smith then applies VASER ultrasound energy to loosen the submental fat pad from the connective tissue around it, and passes a thin cannula through the same incisions to suction the fat away, refining the contour with finer cannulas. The incisions are closed with a stitch or two and a chin compression garment goes on before you leave. Most cases are done awake under local anesthesia, with light sedation available.

How long does chin liposuction take?

Chin liposuction on its own usually takes under an hour. Plan on one to two hours when the jowls, the sides of the neck, or a skin-tightening step such as FaceTite is added, plus time beforehand for marking, photographs, and numbing.

Which liposuction technique is best for the chin?

For most patients the device matters less than the surgeon's control over how much fat is removed and from which layer. Traditional suction-assisted liposuction with a microcannula remains the workhorse for the chin. Tumescent infiltration reduces bleeding and bruising regardless of the device used. Power-assisted cannulas vibrate to loosen fat more evenly, and Dr. Smith pairs them under the chin with VASER, which uses ultrasound energy to separate fat from the connective tissue holding it. He considers VASER beneficial in virtually every liposuction case, because it reduces blood loss and because the tightening it produces in the deeper tissue helps the final contour. What is VASER chin liposuction?

VASER stands for Vibration Amplification of Sound Energy at Resonance. Laser-assisted liposuction, marketed as Smart Lipo, passes a thin laser fiber through 1 to 2 mm incisions to liquefy submental fat and heat the underside of the skin before the fat is suctioned out. Dr. Smith does not offer it. He uses VASER ultrasound energy with power-assisted liposuction, and when skin tightening is part of the goal he adds FaceTite radiofrequency. Laser liposuction does not remove significant loose skin and is not a substitute for a neck lift.

Does Dr. Smith offer J-Plasma (Renuvion) skin tightening?

No. Dr. Smith does not offer Renuvion, also called J-Plasma. When skin laxity needs treating alongside chin liposuction, he uses FaceTite radiofrequency, which heats between a probe below the skin and an electrode above it, through the same small incisions used for the liposuction. It suits mild to moderate laxity; skin that hangs or bands needs a neck lift. Swelling and tightness settle over about two weeks and tightening continues to improve for three to six months.

Candidacy and combining procedures

Is chin liposuction worth it?

For a patient with a localized fat pad under the chin and reasonably elastic skin, chin liposuction is one of the highest-satisfaction procedures we perform: a single treatment, done awake, with a permanent reduction in fat and a visibly sharper angle between chin and neck. It is not worth it if the real problem is loose skin, platysmal bands, or a small chin bone, because removing fat will not fix those and can make them more obvious. Sorting out which of these you have is the point of the consultation. The downsides are temporary swelling and bruising, a chin garment worn for 48 hours and then at night for five nights, and a small risk of contour irregularity.

Chin liposuction or a neck lift: which do I need?

Chin liposuction removes fat; a neck lift removes skin and tightens the platysma muscle. If you can pinch a fat pad under your chin and the skin snaps back when you release it, liposuction alone will likely give a clean result. If the skin stays slack, or you see vertical bands when you clench your jaw, a neck lift (platysmaplasty, often with cervicoplasty to remove skin) is the right operation. A neck lift uses incisions behind the ears and under the chin, is usually done under general anesthesia, and needs two to four weeks of recovery rather than a few days. Many patients need both, done together.

Will chin liposuction tighten loose skin, or do I need a separate skin-tightening treatment?

Liposuction produces modest tightening on its own, because removing fat lets elastic skin redrape over the smaller contour. Under about 40, that is usually enough. Beyond 40, or after significant weight loss, mild to moderate laxity often needs help: Dr. Smith typically pairs the liposuction with FaceTite radiofrequency in the same session, so heat is applied to the underside of the skin while the fat is removed. Morpheus8 can be used afterward for smaller improvements. Treating fat and laxity together avoids a second recovery; staging is reserved for cases where aggressive tightening would add too much swelling.

Am I too old for chin liposuction?

There is no age cutoff; skin quality matters more than the number. Patients in their 20s and 30s have the most elastic skin and see the smoothest redraping. In the 40s and 50s liposuction still works well, often with FaceTite added. Past 60, the skin under the chin has usually lost enough collagen that fat removal alone risks leaving it loose, so Dr. Smith more often recommends liposuction combined with a neck lift, or a neck lift alone. Older patients also heal more slowly, bruise longer, and need a medical review and medication check before an elective procedure.

Is chin liposuction different for men?

The operation is the same; the goal is not. Men usually want a more angular jaw and a crisp angle between chin and neck rather than a softer face, so Dr. Smith removes fat conservatively along the jawline and preserves the fullness that reads as masculine. He also likes to use VASER in men whenever he can, because male connective tissue tends to be tougher and can impede liposuction, and the ultrasound energy loosens the fat from that tissue before it is suctioned. The causes are the same for everyone: genetics, weight fluctuation, age-related skin laxity, a recessed chin, and posture that keeps the neck flexed; men at a healthy weight commonly store fat under the chin. Can chin liposuction be combined with buccal fat removal?

Yes, and the two are often done in one session for a slimmer lower face overall. Buccal fat removal (bichectomy) takes out part of the buccal fat pad through a small incision inside the cheek near the upper second molars, hollowing the area below the cheekbone; chin liposuction sharpens the jawline and chin-neck angle. Buccal removal takes about 30 to 60 minutes and leaves no external scar. Dr. Smith deliberately leaves some buccal fat in place, because over-resection produces a gaunt look as the face loses volume with age. Combined recovery is about a week of swelling with a soft diet and careful oral hygiene; the final cheek contour appears over two to three months.

Can liposuction also slim the cheeks and jowls?

Yes. Facial liposuction treats the lower cheeks, jowls, jawline, and neckline, and is commonly done with chin liposuction in one session. Small incisions are placed under the chin and, when needed, in the crease behind the earlobe, and micro-cannulas remove small, precise amounts of fat along the jaw. Fullness higher on the cheek, immediately beneath the cheekbone, is buccal fat and is better treated with buccal fat removal than with liposuction. Good candidates have pinchable fat with good skin tone; if jowls are mainly sagging tissue rather than fat, FaceTite or a facelift addresses that better. The combined procedure takes one to two hours under local anesthesia with or without sedation.

How is face liposuction different from liposuction elsewhere on the body?

Volume. Body liposuction can remove liters of fat; facial liposuction removes a few tens of milliliters, often 50 to 100 cc under the chin, using very small cannulas. Precision matters more than quantity: removing too much fat from the face, or from the wrong plane, creates hollows and an aged look that is difficult to correct, and the marginal mandibular nerve, salivary glands, and vessels must be avoided. Facial liposuction can be done alone or with fillers, fat grafting, FaceTite, or a facelift.

What is recovery like after face and cheek liposuction?

Expect swelling and bruising in the cheeks and jawline that peak around day three and settle over one to two weeks; the jowl area holds swelling longest. You go home the same day in a compression garment and will need someone to drive you. Most patients take two or three days off desk work, up to a week for physically demanding jobs, and resume normal activities at seven to ten days; strenuous exercise waits at least two weeks. A slimmer contour is visible by about three weeks, most swelling is gone by six weeks, and the face continues to refine for several months. Head elevation, cold compresses, and stopping blood thinners beforehand reduce bruising.

Should I have a chin implant with chin liposuction?

If your chin is recessed, consider it: liposuction alone cannot create a jawline the bone does not support. A chin implant adds forward projection through the same incision under the chin (or one inside the lower lip), while liposuction removes the fat pad above the neck, so the two lengthen and define the profile together. Solid silicone implants are the most widely used because they are easy to position and to remove or exchange; porous implants such as Medpor allow tissue ingrowth, which resists shifting but makes revision harder. Combined recovery is one to two weeks of swelling with a chin garment. Filler along the chin and jawline offers a temporary preview for patients unsure about an implant.

What is submentoplasty, and when is it recommended?

Submentoplasty is chin liposuction plus direct work on the neck through the same incision under the chin: the edges of the platysma muscle are tightened at the midline and a small amount of loose skin may be excised. It is recommended when there is more fat and laxity than liposuction alone can handle but not enough sagging to justify a full neck lift with incisions behind the ears. Recovery is longer than after liposuction alone, typically two weeks of swelling, and the compression garment is worn around the clock for 1 week and then at night only for a second week. That is a longer schedule than chin liposuction on its own, where the garment is worn for 48 hours straight and then at night only for 5 nights. Results, longevity and repeat procedures

Is chin liposuction permanent?

The fat cells removed are gone permanently. Two things can still change the result. Significant weight gain enlarges the fat cells that remain, under the chin and elsewhere, so the neck can thicken again. And aging loosens skin and lets deeper fat pads descend, so a jowly or full look can reappear 10 to 15 years later even at a stable weight. That later change is a skin problem, treated with FaceTite or a neck lift rather than more liposuction. Patients who keep their weight steady and protect their skin from sun typically enjoy the improved profile for a decade or more.

Will chin liposuction give me a defined jawline?

It will reveal the jawline you have. Removing the submental fat pad and the fat that drapes over the jaw border sharpens the angle between chin and neck and lets the edge of the mandible show. Whether that reads as "defined" depends on the underlying bone: a strong jaw and chin look dramatically better; a recessed chin or narrow jaw may look cleaner but still soft, and Dr. Smith may suggest a chin implant, filler, or fat grafting to build the framework. Loose skin blurs the result and is addressed with skin tightening or a neck lift. Filler or neuromodulator injections into the jaw muscles give subtler, temporary changes.

Can you get chin liposuction twice?

Yes, but it is rarely needed and should be approached carefully. A second procedure is reasonable when residual fat or a small asymmetry remains after full healing, or when weight regain has added fat. Dr. Smith waits at least six to twelve months before revising, because swelling and skin retraction are still changing the result until then. VASER is non-negotiable for him in revision liposuction: the ultrasound energy breaks up the scar tissue left behind by the first procedure so the remaining fat can be removed evenly. Repeat liposuction still removes less each time and cannot tighten skin that is already loose; it also carries more risk of irregularity. Why does the final result take months to appear?

Because healing happens in three overlapping stages. In the first two weeks the tissues repair; swelling peaks around day three to five and then falls, and about 90 percent of it is gone by a month. Over the next two months the deeper firmness softens, numbness under the chin fades (usually within one to two months), and the skin retracts onto the new contour. The final shape is judged at three to six months, and the small incision scars fade over six to twelve months. What are the risks of chin liposuction?

Serious complications are rare. Expected effects are swelling, bruising, and temporary numbness. Uncommon problems include contour irregularity or asymmetry from uneven fat removal, over-resection that leaves a hollow, a seroma (fluid collection) that needs to be drained, and infection, which occurs well under one percent of the time. The marginal mandibular nerve runs along the jaw and, if bruised, can weaken the lower lip on one side for weeks; permanent injury is very rare when the cannula stays in the correct plane. How should I prepare for chin liposuction?

Stop nicotine in any form, including smoking and vaping, for 4 weeks before and 4 weeks after surgery; the practice tests for it, and nicotine impairs healing and skin retraction. Stop blood thinners and supplements, including aspirin, ibuprofen and other NSAIDs, fish oil, and vitamin E, 10 days beforehand, and avoid alcohol for two weeks. Never stop a prescription medication without checking with the prescribing physician. Keep your weight stable for 3 months before surgery. Arrange a ride home, help for the first day or two, soft foods, and ice packs. Patients with medical conditions should have clearance from their primary physician. Wear loose clothing on the day, and consider a Friday date if you want to be back at your desk on Monday.
