Hey everybody, welcome back to Plastic Surgery Before and After. This is season 2. We are returning after a several year hiatus and we have a really cool thing to talk about today. It's maybe the most exciting development in aesthetic surgery in the past few years. It's really something totally novel and something we've been hoping to have at our disposal for a very long time and now it's here.

This new really kind of game-changing innovation is AlloClae. AlloClae is an off-the-shelf fat graft. It comes from purified human donor fat and this is an answer for patients that would benefit from fat transfer but don't have fat of their own to donate to the cause. So this is a really exciting option for our fit and thin patients who want a breast augmentation but don't want implants and who would be great candidates for a fat transfer-based breast augmentation with AlloClae.

[02:04] We can actually do a breast augmentation with an off-the-shelf fat graft in under an hour under local anesthesia. Another group of patients that's really benefiting from this are patients that are losing weight with the GLP-1 medications like Ozempic, Mounjaro. These are folks that are losing significant amounts of weight that also, unfortunately, sometimes comes with volume loss where you would rather hold on to it, think the breasts, the buttocks. And these are also patients that usually don't have much of their own fat to donate because of their weight loss. Now, we can go right ahead and add some volume back into the breasts or back into the buttocks to get a fit, toned appearance without having to rely on artificial implants or on fat from the actual patient.

[03:00] The other really cool use of AlloClae is in cases of liposuction revision. So, we get a lot of patients that have unsatisfactory liposuction outcomes from elsewhere and one of the mainstays of treatment for this is fat transfer. However, there are a lot of patients that come to us and they don't have much extra fat on their body to use for this purpose. So, patients that need liposuction revisions and they have things like dents and divots that previously might really have had limited options, AlloClae opens a whole new world of possibilities for them too. This is a really straightforward procedure from the patient's perspective. It does not require anesthesia.

[03:47] You really could go back to remote work the same day as the procedure is performed and it's pretty reasonable to plan to be back in an office setting in about 48 hours so you can come and have this done on a Friday and you're back at it on Monday. And the other thing that's really cool about this is results and volumization is instant. So, the AlloClae that is placed is not actually living fat. It's a 3D structure made up of intact fat matrix from the donor that your body goes in and repopulates with its own fat cells. So, it's almost a scaffolding that allows your body to repopulate it with fat and you end up with nice soft natural fat.

[04:41] And you see the volume right away from this and what's really cool is retention is superb. So, in early experiences, people are reporting retention in the 80 to 90 percent range.
