Ultrasound-Assisted BBL: Fat Placement and Safety

Ultrasound-assisted BBL uses real-time imaging during fat injection so the surgeon can see the cannula’s position relative to the gluteal muscle. It is a guidance technique within a Brazilian butt lift, which transfers your own fat to the buttocks. Fat entering large blood vessels during buttock fat grafting can cause a fat embolism and can be fatal. Fat must remain in the subcutaneous fat above the gluteal muscle. Ultrasound helps the surgeon monitor the cannula’s position during injection; it does not make BBL risk-free. In ultrasonic BBL, a device called an ultrasound transducer (see below) is positioned over the site of fat injection. This device is both a speaker and a microphone. It uses its speaker to send out ultrasound waves (sound waves outside the range of human hearing). It then listens for those waves to come back with its microphone. Since all materials reflect sound differently, a computer can create an image of the area of interest based on how the ultrasound waves are bounced off the tissue targeted by the device. Ultrasound guidance is one part of surgical planning. Health, available donor fat, desired volume and the overall treatment plan still determine whether BBL is appropriate. Some patients with less donor fat may be considered for an “athletic” BBL, using small amounts from several areas for a subtler change. BBL fees are quoted after examination and depend on donor areas, the amount of fat harvested, operating time, anesthesia and any combined procedures. The quote separates surgeon, facility and anesthesia fees; postoperative visits are included. The consultation fee is $500, credited toward a booked procedure. Swelling, bruising and soreness are expected after BBL, especially in the first days. Numbness can occur and a patch of altered sensation may persist. Recovery instructions include avoiding direct pressure on the buttocks for the first two weeks and wearing compression for four weeks after liposuction. Light activity is usually resumed after a few days, while strenuous activity and prolonged direct sitting are avoided for two to three weeks.
